Valid and active license issued by the Physical Theraphy Baord of California.
At least 12 consecutive months of performing similar services within the last 3 years.
CPR and BLS certification from American Heart Association (AHA) or Red Cross (with AED).
Must be proficient in the English Language (Speaking/Writing).
Responsibilities:
Under Institution CEO/CME's guidance, the Physical Therapist duties/responsiblities shall include, but not limited to:
Providing care per licensure scope and Institution/Facility policies, in line with evidence-based Physical Therapy practices.
Offering Physical Therapy and evaluations as needed, based on scheduled appointments by the Institution/Facility CEO/CME or designee.
Delivering Physical Therapy to patients/youth via physician referral, orders, or direct access for documented diagnoses.
Conducting initial assessments, forming treatment plans, and communicating effectively with patients/youth and medical staff.
Completing reports, assessments, and progress notes using CDCR-approved forms in patients' records.
Being reachable for phone consultation during Institution/Facility duty hours.
Maintaining rigorous daily charting; overdue charts beyond ten (10) days might end the Vendor Worker's engagement.
Salary:
$63 per hour.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
